At Back to Basics Health Group, we’re passionate about providing therapy that’s meaningful, measurable, and client-focused. But how do we know if therapy is making a real difference? How do we ensure that our clients are progressing towards their goals?

The answer lies in measuring therapy outcomes. By tracking progress, using structured assessments, and reviewing goals regularly, we can ensure therapy remains effective and adaptable to each client’s evolving needs.

During the Nacre Quarterly Accelerator Conference on 21st March, our Director, Andrew Charalambous, presented on the importance of Measuring Therapy Outcomes. He explored why tracking progress matters, key strategies to improve therapy planning and reporting, and how our multidisciplinary approach ensures clients receive the best possible support. Here’s a closer look at why this matters for our clients and how we put these strategies into action.

Why Measuring Therapy Outcomes Matters for Our Clients

Tracking Progress & Celebrating Achievements
Every client’s therapy journey is unique, and measuring outcomes helps us track improvements, identify challenges, and celebrate successes. By using structured assessments and feedback from clients and families, we can clearly see what’s working and where adjustments may be needed.

Setting and Reviewing Goals
At Back to Basics Health Group, we work closely with our clients and their families to develop individualised therapy goals. These goals are based on the SMART framework (Specific, Measurable, Achievable, Relevant, Time-bound), ensuring they are clear, realistic, and trackable over time.

Regular goal reviews allow us to:

  • Assess progress and make adjustments if needed
  • Ensure therapy remains aligned with the client’s evolving needs
  • Keep both clients and therapists accountable in working towards meaningful outcomes

Supporting a Collaborative Approach to Therapy
Therapy is most effective when all professionals involved in a client’s care are working together. Our multidisciplinary team approach ensures that speech pathologists, occupational therapists, physiotherapists, and other specialists are all aligned in their goals and strategies to support our clients holistically.

By measuring therapy outcomes, we can:

  • Coordinate therapy interventions across disciplines
  • Reduce duplication of services and streamline care
  • Improve communication between therapists, clients, and families

Ensuring Clients Have the Right Level of Support
For many of our clients, accessing NDIS or Aged Care support is essential for receiving ongoing therapy. However, funding is often based on evidence of progress, meaning that up-to-date reports are critical when applying for, reviewing, or maintaining funding.

By keeping clear, consistent records of therapy outcomes, we can:

  • Provide strong evidence for NDIS plan reviews and funding requests
  • Ensure clients continue to receive the level of support they need
  • Prevent funding delays or reductions due to missing or outdated reports

How We Measure Therapy Outcomes at Back to Basics Health Group

Using Standardised & Informal Assessments
We use a mix of standardised assessment tools (such as functional capacity assessments, communication assessments, and sensory profiles) and informal assessments (such as therapist observations and client feedback) to gain a full picture of a client’s progress.

Annual Action Review Plans
Every year, we conduct Action Review Plan Meetings with our clients and their families. These structured sessions help us:

  • Review current goals and assess progress
  • Identify what’s working and what needs adjusting
  • Set new goals to ensure therapy remains relevant
  • Provide documented evidence of progress for funding purposes

Multidisciplinary Team Meetings
Our therapists regularly meet to discuss client progress, share insights, and refine strategies to ensure the best possible outcomes. These collaborative meetings help ensure therapy is well-coordinated and tailored to each client’s unique needs.

Progress Reports & NDIS Readiness
NDIS funding reviews can happen at any time, and clients need to demonstrate ongoing progress to maintain their funding. By keeping accurate progress reports, we ensure our clients are always prepared for plan reviews and reassessments, reducing the risk of funding disruptions.
